Becoming a Patient
The easiest way to become a client at one of the AHF Healthcare Centers is to call our Client Retention Coordinator at 888.AIDS.CARE. We will help you set up an appointment at one of our many convenient locations. To view our various locations in the US, you can click here.
Insurance
- We accept most major insurances, including all PPO plans.
- If you have an HMO you would need a prior authorization from your primary provider, unless you wish to pay cash for your care.
What To Bring To Your First Appointment
- Insurance Card (or evidence of any other coverage)
- Proof of residency (utility bill, lease or rental agreement)
- Proof of income (pay stubs, bank statement with direct deposit entries, award letter or notice of payment from any disability or unemployment benefits)
AND
- If you are HIV positive, bring your:
- Original diagnosis document, or
- Blood test results, or
- HIV medication bottles (with your name on the label)
NOTE: Not sure if you are HIV positive? Get Tested
Your First Appointment
Please arrive to the Healthcare Center half an hour early in order to begin filling out the paperwork. When you arrive you will be greeted by the front office staff and provided with initial forms and information. Please let us know if you need assistance in completing the forms. You will then meet with the Benefits Counselor who will conduct a financial and benefits screening and register you as a client. This will also allow us to determine if you can qualify for any other services such as AIDS drug cost assistance (ADAP) and/or Medi-Cal. On this initial visit you will receive a new client orientation and a tour of the site. We look forward to meeting you. Thank you for giving us the opportunity to be your health care provider.
